Pick a single patient with chronic-disease needs. We assemble the multidisciplinary team around them — disciplines chosen for that patient, clinicians local to your area.
The pilot is fully billable. That is the rebate to the GP for a 40-minute pilot conference, under MBS item 743.
You will not be working any of this out on your own. An experienced clinician from our team is assigned to your practice and stays with you from the booking through to the documentation — choosing the patient, convening the team, running the conference and closing it out.
You coordinate the conference, we bring the clinicians and work with you to make sure everything runs smoothly.

The GP claims the item like any other. Times and participants are recorded as the conference runs, and the patient assigns their benefit by text.
GPs typically convert the conference into a standing schedule to handle their ongoing patient load. Specialists can be brought in on request following the pilot.
Nothing about your patient is discussed without their agreement. You raise it once, in the consult — from there we capture each consent, record it and follow up anything outstanding.
You ask whether the patient agrees to their care being discussed at a case conference. That one agreement covers every clinician who takes part.
We confirm it with the patient beforehand, with the particulars of what will be covered. They do not have to be there.
Afterwards the patient assigns their Medicare benefit by SMS — nothing to print, nothing to sign. That one is ours to chase.
Bookings need at least two weeks' notice so the clinical team can be convened.
The GP presents the patient and leads the discussion, consulting the clinical team on the areas where their input will make a difference. Each clinician contributes their perspective in real time — assessment, recommendations and next steps for their part of the patient's care. Everyone documents as they go: treatment goals and recommendations are captured in a single collaborative record, finalised after the conference and ready for upload to the patient's file. Our clinician-led support team is on hand throughout — guiding where helpful and making sure everything runs smoothly.

No — there is no charge and no ongoing commitment, and payment details are never requested. Conference.care does not take any share of Medicare billings.
A browser with a camera and microphone — that's it. Nothing to install, no accounts to set up beforehand. The confirmation email carries a secure joining link for the GP.
The coordinating GP claims one of three time-tiered MBS items for organising the conference — 735, 739 or 743 ($84.65 to $241.25). Which tier applies follows from how long the conference runs, and that is a clinical decision for the GP. Participating allied health clinicians claim their own time-tiered items (10955, 10957, 10959), and specialists bill theirs separately — the platform prompts the correct item numbers from the recorded duration. Case conferencing is rebated at higher rates than standard consults. The practice bills Medicare directly; Conference.care never takes a share.
The ones that work best for a first conference are the patients you already have a problem with — someone stuck on a long wait list, someone who won’t follow through on a referral, someone you can’t get a specialist for, or the one who keeps coming back no better. If a name came to mind reading that, that is the one.
It helps to pick someone already booked in the next fortnight, so the consent conversation happens at an appointment you were having anyway. And if nobody springs to mind, say so when you book — working out who would benefit most is something our clinician-led team does with GPs every week. There is more on this in choosing a patient.
Any patient with a chronic disease qualifies — commonly diabetes, chronic pain, cardiorespiratory disease, geriatric care, stroke recovery and palliative care. That is a wide net on purpose, which is why the question above is the more useful one.
Three things: choose the patient and a time, have the consent conversation with the patient, and the GP joins the call. The platform makes the rest easy — connecting the allied health team, scheduling, consent records and Medicare-compliant documentation, with a conference summary provided for upload to the patient record.
No. The conference is where the multidisciplinary team comes together — each participant contributes a different kind of care or service to the patient's plan, and may go on to see the patient afterwards where the GP recommends it. No prior consultation with the patient is required.
You'll get a confirmation email straight away with the details, your booking reference and the clinical team — so you can have the right patient in mind. Reminders follow the day before and shortly before the conference, and the joining link is emailed one hour before it begins. The GP receives these as well.
The clinical team's notes are finalised and made available for upload into the patient's file in your clinical package — a complete record of the conference and each clinician's contribution. The practice account also remains in place, so the GP can coordinate their next case conference whenever they're ready.
